L.I.V.E. Lifesaving Information Vial (for) EmergenciesSpeaks for you when you can't! A free program brought to you by the Masons of Haldimand County. In cooperation with Haldimand County Emergency Medical Services. What is the L.I.V.E. Program? It is a proven means for Emergency Personnel to quickly access vital medical information on members of a household during a medical emergency. The kit consists of the following: 1. One plastic vial. 2. Medical information form. 3. L.I.V.E. stickers for your doors and a fridge magnet. How can it save a life? - Emergency personnel will see the sticker on your door. -They will know they can find your vital medical information on the top shelf of your refrigerator door. This information geatly assists them to more accurately asses the patient's needs and administer the correct treatment with less delay.
Should you participate?
L.I.V.E. is designed primarily to serve people who live alone, have restricted mobility or chronic health problems, but is open to everyone in Haldimand County.
Why should your participate?
- Because medical emergencies do happen to anyone at any time, usually at the worst possible times, when family help is not available.
- The L.I.V.E. vial provides medical information quickly to Paramedics allowing them to reduce the time to make informed decisions about how to provide appropriate treatment for you.
- If you are unable to provide important medical information during an emergency, how will Paramedics be able to treat you promptly? If all of your medical information is available and located in a central location (your refrigerator door), precious moments can be saved by those treating you, including nurses and doctors at your local hospital.
How L.I.V.E. Works
* Complete the information sheet found on the inside of the L.I.V.E. vial. Be sure to enter the date that the information sheet was completed. Complete one side of the form for each person in the residence. If there are more than two people, use one vial for every two people.
* Attach one of the self adhesive labels to the upper left corner of your front, and one on the upper left corner of your back door, and put the fridge magnet on the upper left corner of your refrigerator door.
* Put the vial with the completed information sheet inside, on the upper shelf of your refrigerator door.
* Haldimand Emergency Personnel (Paramedics) have been trained to look for the L.I.V.E. stickers on the doors of a residence, and will know that the residents have valuable medical information in their refrigerator.
Where?
Vials are available at all Pharmacies in Haldimand County and all Haldimand County Libraries and Offices.
Remember!
It is very important that the information on the sheet be kept up to date as your medical information and medications change. You can write the information in pencil and change it as necessary or obtain a new sheet from a Pharmacy, Library, Town Office or by downloading one from:

No medications are to be kept in the vial.
The "forget me knot" stickers must be on your doors for Emergency personnel to know that the L.I.V.E. vial is in your home.
The information sheet asks you to list a person to contact in the case of an emergency. Please keep this information up to date, especially the phone number.
If you know of a person living alone, or with special needs, tell them about the L.I.V.E. Program, and tell them where it is obtainable.

The stylized "forget-me-not" was selected as the symbol for the L.I.V.E. Program as it has been a reminder to Masons everywhere, to be ever mindful of the needs of others. the program was developed in partnership with the Haldimand County Emergency Medical Services.
Emergency Personnel have been trained to look for the symbol at the top left corner of your outside door. It informs them that critical medical information is available in your refrigerator. Even if the colour fades, the distinctive shape of the symbol will remain to tell them, that you have put important medical information in your refrigerator.
